CST Facilitator Application

This application is for COA staff members who are proposing to complete CST Facilitator Training as part of the UMass FIRST initiative and facilitate two 16-session program cycles for groups of people who have been diagnosed with mild to moderate dementia.

CaringKind, the non-profit organization providing the CST Facilitator Training for selected trainees, will offer two two-hour sessions a week for three weeks, totaling 12 hours. It is anticipated that training will be completed by late April 2026.

Once training is completed, facilitators may begin implementing CST cycles, with a requirement to complete two cycles of CST by May 1, 2027.

If you are not a COA director, does your director fully support involvement in the UMass FIRST Initiative and understand the details of the commitment, which includes leading two 16-session program cycles with a co-CST facilitator between the completion of CST training in early 2026 and May 1, 2027?
Do you have experience working with people living with dementia?
Do you have experience leading small group discussions or trainings?
Have you collaborated with other COAs, municipal departments, or community partners on past programs or projects?
Please note if you have the capability to offer the CST program in-person, virtually, or both. CST session cycles can be held either in-person or virtually, but cannot be delivered via a hybrid model.
