CST Facilitator Application

This application is for COA staff members who are proposing to complete CST Facilitator Training as part of the UMass FIRST initiative and facilitate two 16-session program cycles for groups of people who have been diagnosed with mild to moderate dementia.

CaringKind, the non-profit organization providing the CST Facilitator Training for selected trainees, will offer two two-hour sessions a week for three weeks, totaling 12 hours. It is anticipated that training will be completed by late April 2026.

Once training is completed, facilitators may begin implementing CST cycles, with a requirement to complete two cycles of CST by May 1, 2027.

Trained facilitators are required to offer two CST program cycles between May 2026 and May 2027. Please provide an estimated timeline for when you envision offering two program cycles to your community in partnership with a CST facilitator from another COA. *
